Appointment Information:
- Your initial appointment with us, New Evaluation, or for a new diagnosis we have not treated, will be approximately 60 minutes.
- Please arrive 10 minutes before your New Evaluation appointment time to complete our Registration forms.
- Please bring the following items, as it applies to you, with you to your initial appointment: insurance card(s), ID or Driver's license, physician referral, list of medications.
- Follow up treatments are approximately 30-40 minutes
- As a courtesy, we provide automated appointment reminders via text or email. The reminder will be sent the day before your appointment date.
- There is a $45 fee for a missed appointment or cancellation with Less than 24 hours notice billed to the patient.
Insurance Types Accepted:
- Workers Compensation
- Medicare
- Most Major PPOs except Aetna. Please note: We are in-network with Anthem BlueCross for O.T. Hand Therapy services, however out of network for Acupuncture.
- HMO's: Brown & Toland-East Bay, Sutter East Bay Medical Foundation, and Affinity Stanford Health

We offer discounted rates for patients seeking services without insurance.
As a courtesy, our office will verify benefit coverage with your insurance. We can not quote or guarantee the coverage information given. It is your responsibility as the insured to be fully informed of your insurance group plans coverage for you.

The office is located upstairs with no elevator access. If you have difficulty with stairs, please inform our office.
Non-metered street parking is available on Fairmount Ave and all cross streets.
Bart: Exit El Cerrito Plaza Station. We are 3 blocks West (towards San Pablo Avenue) of the station.
